A new model for clinical case management ensures personalized
service and individual responsibility for each of our young residents.
Licensed clinical social workers, and other masters prepared staff,
conduct psychosocial assessments of youth during their intake to
determine the most appropriate action strategies and goals. Youth
select from a Choices Plan that offers tiered levels of service
ranging from overnight stays to self-sufficiency. They are then
assigned to one of two case management teams led by masters-level
staff, one for young men and one for young women and children.
Each resident works with their team to develop goals
focusing on one or more areas of improvement such as return to school
or obtaining and retaining employment. The Team remains the primary
contact for youth throughout their stay and for a year of aftercare.
Ongoing counseling, group therapies and a pastoral ministry add
to a seamless network of support for our youth
